How often do you have the following experiences? Answer based on your experience when you are not under the influence of alcohol or drugs.
1. The world around me feels unreal, dreamlike, or foggy, as if there is a veil between me and my surroundings.
2. I feel disconnected from my own body, as though I am watching myself from outside or as if my body does not belong to me.
3. I find myself staring blankly into space, thinking of nothing, and not being aware of the passage of time.
4. Familiar places or people look strange, unfamiliar, or unreal to me.
5. I feel emotionally numb or detached, as though my emotions have been turned off.
6. I look in the mirror and do not recognize myself, or my reflection looks unfamiliar or strange.
About This Derealization Test
This screener is adapted from the Dissociative Experiences Scale II (DES-II), developed by Carlson and Putnam (1993). The DES-II is the most widely used screening tool for dissociative symptoms in research and clinical practice. Items selected here focus specifically on derealization and depersonalization experiences.
Derealization is more common than most people realize. Roughly 50% of adults have experienced at least one brief episode. Persistent or distressing derealization may be a feature of depersonalization-derealization disorder, PTSD, anxiety disorders, or depression. Treatment typically involves psychotherapy, particularly CBT and grounding techniques.
